People receiving services in our Community Mental Health Center may get a referral to receive assistance accessing a medical provider, to InSHAPE, and to our in-house tobacco cessation services.

 

Access to Medical Care

Physical health and mental health go hand in hand. If individuals need to establish healthcare with a primary care provider or needs to go in for a long overdue checkup, we help make that happen.

 

InSHAPE Individualized Self Health Action Plan for Empowerment

InSHAPE is a comprehensive, evidence-based health and wellness program designed to improve the health outcomes and social inclusion of individuals living with serious mental illness. People enrolled in InSHAPE will receive:

• Nutritional education and support
• Certified personal training by qualified Health Mentors
• Opportunities to learn healthy cooking skills
• Activities that enhance social connection
• Smoking cessation support
• Access to gyms and other exercise facilities

 

Smoking Cessation

With coordinated supports individuals with mental illness who are considering quitting smoking can successfully do so without negatively impacting their mental health recovery. Individuals may request a referral to receive assistance to quit smoking. Together with our Certified Tobacco Cessation Specialist individuals will receive:

• Tailored assistance to set a quit date
• Learn and practice skills important to their success
• Receive on-going support while quitting
• Assist with nicotine replacement therapy or other cessation prescriptions
